LTE Independent Living Support Pilot Program Specialist
Part time position (50%), 18.75 hrs/wk, with the Aging & Disability Resource Center of Columbia County located within Columbia County Health & Human Services. The Independent Living Support Pilot Program Specialist provides short-term service coordination for individuals participating in the Pilot which ends March 31, 2025. The ILSP Specialist duties may include providing information and assistance via telephone and in person, care plan development, case management, data reporting, documentation, networking with community organizations, and promoting ADRC services. An Associates and/or four-year degree, preferably in the human services field and one year of work experience with persons living with disability and/or elderly populations. Hire rate: $25.00/hr.
