POSITION DETAILS: *Full-Time opportunity *Experienced in Palliative Care (Home Health/ Hospice) *Consulting at home patient visits, working on disease management and pain management of patients. *Energetic, self-starter who can work independently and contribute to team environment *Feel comfortable in a range of settings: small group office and large group organizational setting *Strong Electronic Medical Record skills (EPIC) QUALIFICATIONS: *Valid CA Physician License *M.D. or D.O.
