JOB OVERVIEW
As one of the oldest medical schools in the country, the UC College of Medicine has a reputation for training best-in-class health care professionals and developing cutting-edge procedures and research that improves the health and clinical care of patients.

The Department of Internal Medicine, Division of Endocrinology, Diabetes & Metabolism is seeking a Nurse Practitioner to assist in providing comprehensive, continuous, and personalized care to patients.  This position will work with the Sweet Transitions Program, the goal of which is diabetes management of patients recently discharged from University of Cincinnati Medical Center (UCMC) with A1c ≥ 9% and coordination/communication of diabetes care with patient's PCP or endocrinologist and the patient's "medical neighborhood".

In addition, the Sweet Transitions program Nurse Practitioner will work collaboratively with the Diabetes Now Nurse Practitioner. The persons filling these two roles will each devote about 20%-30% of effort to the other role.

ESSENTIAL FUNCTIONS
*Conducts focused diabetes visit(s) with patients with A1c of 9% or above. Visit is conducted with the collaboration of diabetes educator. *Utilizes preventative clinical guidelines, appropriate comprehensive standards of care as designated by the ADA (American Diabetes Association), and collaboration with endocrinologist/diabetologist in providing quality care to patients presenting with acute and chronic high and low blood glucoses as well as their complications. *Conducts a comprehensive assessment of patients' medical, physical and biopsychosocial status and identifies barriers to improved healthcare. *Utilizes case management model of patients to include performing physical assessments, engaging in therapeutic problem solving, counseling, consultation, and providing any additional support that may be needed by the Sweet Transition patient population. *Along with Sweet Transition team members, tracks patient data including A1c after 90 days and one year, as well as 30 day readmit rate. *Serves as a resource to other members of the healthcare team, providing consultations and education as needed. *Demonstrates leadership through continued development and coordination of the Sweet Transitions program in collaboration with other members of the Sweet Transitions and Diabetes Now transdisciplinary team. *After helping patient to improve glycemic control, transitions patient to primary care or endocrine practice as appropriate. *Works collaboratively with Diabetes Now Nurse Practitioner and shares minor responsibility with Diabetes Now Nurse Practitioner (roughly 20%) towards inpatient Diabetes Now program.

REQUIRED EDUCATION
Master's Degree in Nursing and graduation from an ANA approved Nurse Practitioner Program.

REQUIRED TRAININGS/CERTIFICATIONS
Current/valid RN and APRN license to practice in State of Ohio with prescriptive authority, prior to start.

REQUIRED EXPERIENCE
*Three (3) years of nursing experience. *Three (3) years of experience as practicing APRN *Three (3) years of experience in diabetes management. *Engagement in and enthusiasm for interdisciplinary team work

ADDITIONAL QUALIFICATIONS CONSIDERED
*Two (2) years experience working with patients that have complex medical issues. *Experience as bedside nurse in the acute care setting. *Experience in education of nurses and/or other healthcare professionals. *Comfortable with conducting shared medical appointments. *Comfortable with public speaking and presenting in front of large groups. *Basic familiarity with database/Excel management. *Likes being part of multi-disciplinary team.

*Office environment/no specific unusual physical or environmental demands.
