The MLTC Clinical Manager provides supervision and management for assigned multidisciplinary teams that are responsible for implementing the Managed Long Term Care (MLTC) Program. The Clinical Manager supervises nursing, social work, administrative and other professional staff who are directly involved in the assessment of member care needs, formulating and implementing the patient centered service plan. This position is responsible for  all care management and utilization management activities associated with assigned teams.  The Clinical Manager ensures that appropriate workflows and processes are implemented in accordance with regulatory requirements and policies and procedures using available automated systems.  The clinical manager  participates in plan audits, conducts staff meetings, completes performance appraisals and assists in program monitoring; identifies and assists in resolving barriers to successful implementation of the MLTC Program. This position actively participates in the development of policies, procedures and workflows; monitors productivity and staff effectiveness; assists in monitoring quality outcomes, financial targets and overall program effectiveness.
This position requires:
Bachelor degree in nursing, Master's degree preferred
Current active license/registration in New York State as applicable
Medical management, case management or quality management experience in a health plan
5 years of clinical practice in adult or geriatrics, preferably at least 2 years in an outpatient setting
Minimum of 2 years of supervisory experience
Knowledge of:
Case Management concepts
Quality/ Performance Improvement practices

Medicare and Medicaid benefits and guidelines
