Care Team Navigator Lead - The EDGE

The role requires experience working with people in need and providing them exemplary customer service. As a key member of the Edge Operations and Clinical team, you will have the chance to bring your bright ideas and energy to work every day to improve the lives and health of the seniors we serve. We are looking for a person who is willing to jump in and help build this business from the ground up, contributing wherever assistance is needed.
The Navigator Lead for the Edge Care team will serve as the cross functional lead between the clinical care teams and the service teams to develop, lead and guide our members, helping them understand their benefits, helping them to identify what social services they quality for and getting them set up with those resources, coaching members to help them achieve their health goals and connecting them with other members clinical of the care team as appropriate. This role is responsible for the daily activities across multiple functional service areas. He/she requires an in-depth understanding of how organization capabilities interrelate across the function or segment.
The Navigator Lead may perform configuration, installation, implementation, client support and client services related to the care management platform and system. Decisions are typically related to the implementation of new/updated programs of large-scale projects for the function and support of technical/operational procedures and processes. Implements strategic plans, drives goals and objectives, and improves performance. In the role you will provide strategic input and decision making in the assigned functional service areas.
Strong program management, process management and organizational skills
Proficiency in Excel and PowerPoint
Excellent oral and written communications skills
Proven success in using influencing skills to achieve impact and results
2-3 years of experience in the field of service operations in the areas of Enrollment, Premium Billing , Plan Networks, Provider Contracts, Provider Credentialing, Provider Connectivity, Issue Resolution, Provider Capitation, Risk Financial Management, Patient Navigation and/or Social Service Provision.
5 years of management experience of 5 or more associates
Qualified candidate MUST reside in Boston, MA
Ability to travel to Greenville, SC
Master’s in Business Administration or related field
Project Management experience
Experience supporting individuals through behavior change including motivational interviewing and self-management
Experience working with seniors and/or caregivers
Scheduled Weekly Hours
