Bilingual Case Manager (Spanish is a MUST)


Requirements:
This position is responsible for coordinating all social services for residents, including working interactively in the development of an Independent Living Plan. The principal focus of the case manager is to assist the resident in becoming empowered toward the attainment of independent living.
Bachelor’s Degree (Bachelor’s of Social Work preferred) and three years demonstrated experience with homeless populations; or five years demonstrated experience with homeless populations.
Demonstrated case management skills.
Excellent oral and written communications skills.
Ability to keep calm and work well with people.
Good judgment and problem solving ability.
Demonstrated organizational skills.
Responsibilities
Social Services:
Meet with the resident bi-weekly to review progress and identify any obstacles.
Assist in developing permanent re-housing plans.
Providing crisis intervention counseling.
Entitlement referrals and advocacy.
Linking residents to agencies which provide services for special needs and assist in other areas of concern.
Accompanying residents to appointments and/or service providers, as needed.
Assist in ensuring the rules and regulations are communicated to the residents.
Co-facilitating monthly house meetings and floor meetings, weekly employment and specialty groups and workshops, and other employment programs.
Development and implementation of employability plan with participant.
Must be able to work evening and holidays.
Records Maintenance:
Responsible for preparing all documentation relevant to referrals and services provided to residents.
Ensure documentation is in compliance with agency requirements, and maintain privacy and confidentiality of residents.
Statistics and Data Reports:
Responsible for logging and preparing statistical and demographic reports for services provided as required, as well as maintaining any database or computerized records. Reports will include the following:
Update of SCIMS
Monthly statistical reports
Any other activities related to job.
